Attention parents of 2015-2016 6th graders:
As the parent of an upcoming 6th grader, Nurse Kandy
wants you to be aware that all students who will be eleven years old on or by
August 3, 2015, are required to provide a current Alabama Certificate of
Immunization or “blue card” containing proof that a dose of Tdap vaccine has
been received. Tdap is a tetanus, diphtheria and acellular pertussis vaccine.
If you have already provided an updated blue card
containing this information, thank you very much.
If you have not, please provide an updated “blue card”
containing written proof that your student received the Tdap vaccine as soon as
possible.
We expect to have 2015-2016 class schedules ready the
last day of school. Students who have provided their updated blue card will
receive their schedules at that time.
Students without updated blue cards will have their
schedules held until an updated card is provided.
The first student day of the 2015-2016 school year is
Monday, August 3.
Thank you for taking care of this state requirement as
soon as possible.
We are planning now and looking forward to the 2015-2016
school year.
